The physicians of Glenwood Medical Associates are considered to be 'In Network' or 'Participating Providers' for the following healthcare insurance groups:
- Anthem Blue Cross (does not include HMO and POS plans)
- Choice Care / Humana
- Great-West Healthcare
- Medicare
- Pacificare - PPO & SDHP
- PHCS
- Rocky Mountain Health Plans - HMO, PPO, ASO & Medicare
- Roaring Fork Valley Community Health Plan
- Aetna/Sloan's Lake Managed Care - Must have SLMC logo on insurance card
- The Colorado Network
- United Healthcare (effective February 1, 2007)
If you are a beneficiary of one of the above listed groups, we do require a copy of your card and a completed "Contract Insurance Set-up Sheet".
