• Anthem Blue Cross (does not include HMO and POS plans)
• Choice Care / Humana
• Great-West Healthcare
• Medicare
• Pacificare - PPO & SDHP
• PHCS
• Rocky Mountain Health Plans - HMO, PPO, ASO & Medicare
• Roaring Fork Valley Community Health Plan
• Aetna/Sloan's Lake Managed Care - Must have SLMC logo on insurance card
• The Colorado Network
• United Healthcare (effective February 1, 2007)

If you are a beneficiary of one of the above listed groups, we do require a copy of your card and a completed "Contract Insurance Set-up Sheet".
