GMA works with a wide range of insurance plans to help make your care more accessible and affordable. We currently accept:

Aetna: PPO only

Anthem Blue Cross: HMO and PPO

Cigna/Great West: PPO only

Cofinity: PPO only

Medicare

Rocky Mountain: HMO, PPO and CHP+

UnitedHealth Care: PPO and HMO

Important insurance requirements

If you are enrolled in a Bronze Valley Plan or HMO plan, your insurance requires you to select a GMA provider as your Primary Care Provider (PCP) before your appointment. This step is necessary to ensure your visit is covered. If a PCP is not selected, your plan may deny coverage unless you obtain a referral from the PCP assigned by your insurer.
